Wordpress will ask for your server name, FTP username, and FTP password. If you do not know this information please contact your web host. Note: Not all servers allow for FTP from a web program (meaning PHP is not compiled with FTP enabled). If this happens, please contact your web host or the Wordpress forums for additional information on other plugin installation methods.

Once you have entered your information, hit Proceed.
